It manifests 5–10 years after radiation therapy as progressive atherosclerotic stenosis of the ICA. Vertebral artery involvement is not characteristic. This report describes a patient who received radiation therapy for laryngeal cancer at age 18, developed transient ischemic attack after 7 years, and experienced an ischemic stroke in the left middle cerebral artery territory 8 years post-treatment. Examination revealed left ICA occlusion and right ICA stenosis that progressed over 2 years without statin therapy. The patient condition remained stable for 13 years, no control examination of the neck arteries was performed. Absence of transient ischemic attack or stroke during this period suggests that vertebral arteries, which provide cerebral blood flow in severe ICA lesions, remain unaffected by the pathological process. This phenomenon is likely attributable to their anatomical course within the bony canal, providing protection from radiation damage.</p></abstract><trans-abstract xml:lang="ru"><p>Постлучевое поражение внутренней сонной артерии (ВСА) является поздним осложнением радиационной терапии, проводимой по поводу онкологических заболеваний шеи. Оно проявляется через 5–10 лет после лучевой терапией и представлено нарастающим атеросклеротическим стенозом ВСА. Поражение позвоночных артерий не характерно. В работе представлен больной, перенёсший в 18 лет лучевую терапию по поводу рака гортани, у которого через 7 развилось преходящее нарушение мозгового кровообращения, а через 8 лет — ишемический инсульт в бассейне левой средней мозговой артерии. При обследовании выявлена окклюзия левой ВСА и стеноз правой ВСА, степень которого без приёма статинов через 2 года наросла. В течение 13 лет состояние оставалось стабильным, контрольное исследование артерий шеи не проводилось. Отсутствие преходящего нарушения мозгового кровообращения и ишемического инсульта за этот период предполагает, что позвоночные артерии, которые в условиях выраженного поражения ВСА осуществляют кровоснабжение головного мозга, в патологический процесс не вовлекаются.
