A significant prevalence in the number of atherosclerotic plaques in the cerebral arteries, including those causing stenosis of the lumen by 50% or more, was found in the case of T2DM compared to cases without T2DM. Cerebral infarctions in T2DM were mostly of the multiple nature that was determined by a combination of fresh or organizing extensive, large or medium, infarction with one/several organized medium, small deep, and small surface infarctions. More severe atherosclerosis of the cerebral arteries in T2DM caused a higher rate of infarctions due to occlusive atherothrombosis, arterio-arterial embolism, and tandem atherostenosis in these patients compared to cases without T2DM. In the absence of T2DM, infarctions occurred more often due to embolism of the cerebral arteries from the heart with coronary heart disease compared to cases with T2DM.</p></abstract><trans-abstract xml:lang="ru"><p>При сопоставлении результатов исследования 20 секционных случаев с ишемическими инсультами, возникшими при атеросклерозе, и 20 секционных случаев с ишемическими инсультами на фоне атеросклероза в сочетании с сахарным диабетом 2-го типа (СД2) получены данные, свидетельствующие о высокой значимости СД2 в развитии тяжелых изменений церебральных артерий и обусловленных ими инфарктов мозга различной величины и локализации. Установлено значительное преобладание количества атеросклеротических бляшек в церебральных артериях, в т.ч. обусловливающих стеноз их просвета на 50% и более, при СД2 по сравнению со случаями без СД2. Инфаркты мозга при СД2 носили преимущественно множественный характер, определяемый сочетанием свежего или организующегося обширного, большого либо среднего инфаркта с одним/несколькими организованными средними, малыми глубинными и малыми поверхностными инфарктами. Более тяжелый характер атеросклероза церебральных артерий при СД2 определил более высокую частоту инфарктов, обусловленных обтурирующим атеротромбозом, артерио артериальной эмболией и тандемным атеростенозом у этих больных по сравнению со случаями без СД2.
