Factors influencing the rate of progression and disease outcome remain insufficiently studied. Comorbidity in ALS is of particular interest; however, its structure and impact on disease progression are underexplored.</p> <p>Objective: To study the structure of comorbid pathology, injuries, and surgical interventions in patients with ALS and to assess their impact on disease course.</p> <p>Results. Data from 336 patients with ALS were analyzed. The most common comorbid conditions were diseases of the circulatory system (58%), with hypertension being predominant (41%). Thyroid diseases were identified in 6.8% of patients, mainly in those with bulbar-onset ALS. Approximately 25% of patients reported significant psychoemotional stress within one year prior to disease onset. A negative impact of previous kidney surgeries on overall survival was established. Patients with spinal-onset ALS were significantly more likely to have undergone surgical interventions in general and spinal surgeries in particular within 10 years before symptom onset compared to patients with bulbar-onset ALS. The Charlson Comorbidity Index demonstrated a statistically significant association with both the rate of ALS progression and patient survival.</p> <p>Conclusion. Comorbid pathology is widespread among patients with ALS. Concomitant diseases and surgical interventions may exert a differential influence on various forms of the disease. The most substantial negative impact on ALS progression is exerted by the cumulative burden of somatic disorders, highlighting the importance of comprehensive assessment of comorbidity in the management of this patient group.</p>
<trans-abstract xml:lang="ru"><p>Введение. Наиболее существенное негативное воздействие на течение БАС оказывает совокупное бремя соматических расстройств, что подчёркивает важность комплексной оценки коморбидного фона в ведении данной категории пациентов.</p></trans-abstract><kwd-group xml:lang="en"><kwd>amyotrophic lateral sclerosis</kwd><kwd>motor neuron disease</kwd><kwd>comorbidity</kwd><kwd>Charlson Comorbidity Index</kwd><kwd>prognosis</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>боковой амиотрофический склероз</kwd><kwd>болезнь двигательного нейрона</kwd><kwd>коморбидность</kwd><kwd>индекс коморбидности Чарлсона</kwd><kwd>прогноз</kwd></kwd-group><funding-group><award-group><funding-source><institution-wrap><institution xml:lang="en">Bashkir State Medical University</institution></institution-wrap><institution-wrap><institution xml:lang="ru">Башкирский государственный медицинский университет</institution></institution-wrap></funding-source></award-group><funding-statement xml:lang="en">This work was supported by the Bashkir State Medical University Strategic Academic Leadership Program (PRIORITY-2030)</funding-statement><funding-statement xml:lang="ru">Работа выполнена за счёт средств Программы стратегического академического лидерства Башкирского государственного медицинского университета (ПРИОРИТЕТ-2030)</funding-statement></funding-group></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Кутлубаев М.А.
