At the present time Balo’s sclerosis is discussed in case of MRI detection of two or more concentric areas of demyelination, one or more ring of that is enhanced by the contrast. For 100-year period of the study there are 4 main theory of pathogenesis of Balo’s sclerosis: theory of concentric demyelination, theory of distal oligodendrocytopathy, colloid theory and theory of astrocytopathy. None of the theories includes all details of the disease. One of the details is concentric lesions in monophasic Balo sclerosis and in Balo-like syndromes in chronic demyelinating disorders. In case of Balo’s sclerosis and Balo-like syndromes probability of resistant to immunosuppressiv therapy is very high. So glucocorticosteroid therapy must be initiated right after concentric demyelination determination on MRI. In case of ineffectiveness of glucocorticosteroid therapy plasmapheresis and cytostatic therapy could be applied.</p> <p> </p></abstract><trans-abstract xml:lang="ru"><p>Под склерозом Бало принято понимать монофазное демиелинизирующее заболевание, характеризующееся формированием в белом веществе головного мозга концентрических очагов. В настоящее время склероз Бало обсуждается как диагноз при выявлении на МРТ двух и более концентрических участков демиелинизации, накапливающие контрастное вещество по одному и более кольцу или его части. За вековой период изучения склероза Бало в литературе изложены основные четыре теории патогенеза концентрического склероза: теория концентрической ремиелинизации, теория дистальной олигодендроцитопатии, коллоидная теория и теория астроцитопатии. Однако ни одна из теорий не дает полного объяснения всех особенностей заболевания. Одной из таких особенностей является возможность формирования концентрических очагов как при монофазном склерозе Бало, так и при Бало-подобных синдромах в рамках очередного обострением хронического демиелинизирующего заболеваия. Как в случае склероза Бало, так и Бало-подобных синдромов, высока вероятность устойчивости к иммуносупрессивной терапии, в связи с чем лечение необходимо начинать сразу после выявления концентрических очагов демиелинизации.
