Hand palsy with fine motor skills damage influences greatly on quality of life, because hand function plays an important role in everyday activities and working practices. Thus the aim of the study was to investigate functional and structural features of hand palsy in homogenous sample of MS patients during the relapse and in three months. Data were acquired from 25 relapsing-remitting MS patients during relapse also characterized by unilateral light hand palsy, and in three months. 12 age-matched healthy controls entered the control group. All patients underwent neurologica examination, including nine-hole peg test and dynamometry, and magnetic resonance imaging of cervical spine and brain, including functional MRI and diffusion tensor imaging (DTI) sequences. All patients had lesions in spinal cord on cervical spine MRI. fMRI data analysis during relapse showed differently directed activation changes and allowed to divide all patients in two subgroups. These subgroups differed from each other and control group by deep sensitivity and DTI values. In three months patients in the first subgroup had less complete fine motor skills recovery, which was accompanied also by preservation of activation changes and deep sensitivity asymmetry in comparison with the second subgroup. Received data allow us to assume that clinically similar palsy, caused by spinal cord lesion, could be characterized by greater or lesser sensation impairment contribution. In three months heterogeneous structural and functional changes were observed, depending, possibly, on initially different pathologic mechanisms. Revealed differences should be taken into consideration while rehabilitation planning.</p></abstract><trans-abstract xml:lang="ru"><p>Характерными симптомами при рассеянном склерозе (РС) являются двигательные нарушения, неизбежно приводящие на определенном этапе заболевания к инвалидизации, в т.ч. из-за несостоятельности/истощения адаптивных компенсаторных механизмов при обширных структурных повреждениях. При этом парез кисти с нарушениями мелкой моторики значительно влияет на качество жизни пациентов, т.к. отражается на повседневной активности и профессиональных навыках. В связи с этим целью работы являлось проведение динамического клинико-структурно-функционального анализа пареза кисти у однородной группы пациентов с ремиттирующим рассеянным склерозом. В исследование были включены 25 пациентов в возрасте 19–50 лет с ремиттирующим рассеянным склерозом во время обострения, проявлявшегося в т.ч. односторонним легким парезом кисти. Осмотр в динамике выполнен через 3 месяца (в стадии развития стойкой ремиссии). 12 здоровых испытуемых в возрасте 23–30 лет составили контрольную группу. Всем пациентам проводили неврологическое обследование. Как пациентам, так и контрольной группе была проведена магнитно-резонансная томография (МРТ) спинного мозга и головного мозга, включавшая в себя последовательности для получения данных функциональной МРТ (фМРТ) и диффузионно-тензорной МРТ (ДТ-МРТ). При анализе МРТ у всех пациентов развитие двигательных нарушений было обусловлено очагами демиелинизации в спинном мозге на уровне шейного отдела позвоночника (в головном мозге по ходу кортикоспинального тракта очагов не было). При обработке данных фМРТ во время обострения были получены разнонаправленные изменения активации коры головного мозга, что позволило разделить пациентов на две подгруппы, отличавшиеся между собой и от контрольной группы также по данным ДТ-МРТ и результатам исследования глубокой (вибрационной) чувствительности. Через три месяца во время ремиссии было обнаружено достоверно менее полное восстановление мелкой моторики в подгруппе с асимметричным снижением вибрационной чувствительности на стороне пареза, сопровождавшееся также меньшим восстановлением активации коры.Полученные данные позволяют предположить, что реорганизация коры при клинически сходном парезе определяется не только эфферентным, но и афферентным компонентом. Гетерогенность обнаруженных структурно-функциональных изменений при регрессе двигательных нарушений сопряжена с различными исходными патофизиологическими механизмами и отражается на степени восстановления функции.
