Patients with ET had bilateral distribution of predominantly postural-kinetic tremor with slight prevalence in one hand, and limb tremor often combined with head tremor. PD was characterized by asymmetric debut of clinical manifestations, as well as by rest and postural tremor with slight hypokinesia and rigidity. The frequency of rest and postural tremor was significantly higher than in the PD group. Stabilometry revealed improvement of postural stability in “Target-test” in patients with PD. In the dynamic “Evolventatest” patients with PD showed smaller statokinetic curve length and lower center-of-motion velocity compared to healthy subjects. No stabilometric changes were found in ET patients. In contrast to ET, patients with PD were characterized by significant increase in latencies of saccadic eye movements and fraction of multi-step saccades; eye movements lateralization was revealed on stage I of PD. Discussed is significance of the obtained data for clinical practice.</p>  <p> </p> <p> </p></abstract><trans-abstract xml:lang="ru"><p>C целью поиска дополнительных методов дифференциальной диагностики ранних стадий болезни Паркинсона (БП) и эссенциального тремора (ЭТ) проведено клиническое, психометрическое, треморометрическое, стабилографическое и электроокулографическое обследование двух групп пациентов и контрольной группы сходного возраста. У пациентов с ЭТ наблюдали билатеральное распределение дрожания (преимущественно постурально-кинетического) с незначительным преобладанием выраженности в одной руке, часто встречалось сочетание тремора конечностей с тремором головы. Для пациентов с БП был характерен ассиметричный дебют клинических проявлений, тремор покоя и постуральный тремор, выявлена незначительная гипокинезия и ригидность. Частота тремора покоя и постурального тремора в группе пациентов с ЭТ была достоверно выше, чем в группе БП. Стабилографическое обследование выявило увеличение показателя «качество функции равновесия» в тесте «Мишень» у пациентов с БП. В динамическом тесте с «эвольвентой» пациенты с БП демонстрировали меньшую длину статокинезиграммы и более низкую скорость перемещения центра давления по сравнению с контрольной группой. При ЭТ стабилографических изменений не выявлено. В отличие от пациентов с ЭТ, для БП было характерно достоверное увеличение латентных периодов саккадических движений глаз и доли мультисаккад; на первой стадии БП выявлена латерализация глазодвигательных нарушений.
