In the anamnesis: microsurgical removal of the HIVD at the levels L4–L5 and L5–S1. According to magnetic resonance imaging (MRI), a disc herniation and according to functional radiography – signs of instability were revealed at the level of L3–L4. In order to prevent further progression of the degenerative cascade, the patient underwent surgical intervention – dynamic transpedicular fixation with nitinol rods, a metal with shape memory effect. At pre- and postoperative periods we performed clinical evaluation (VAS pain scale, Oswestry quality of life assessment, modified Macnub scale), MRI of the lumbar spine (an analysis of the fixed and adjacent segments degeneration extent according to Pfirman classification), and functional radiography (estimation of the range of motion in the fixed and adjacent segments).</p> <p><bold>Results. </bold>We observed a significant reduction in back pain, a complete regression of leg pain and an improvement in the quality of life compared to the preoperative level. Twelve months after the surgery, on the MRI scan we observed signs of rehydration and regeneration on a fixed segment and the absence of acceleration of degeneration on adjacent segments</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение. </bold>Представлен клинический случай пациентки с распространенным дегенеративным поражением поясничного отдела позвоночника в сочетании с клиническими и радиологическими признаками синдрома смежного диска.</p> <p><bold>Материалы и методы. </bold>Пациентка обратилась в нейрохирургическое отделение ФГБНУ НЦН с жалобами на боли в пояснице с иррадиацией по наружной поверхности левой ноги, онемением в данной зоне. В анамнезе – микрохирургическое удаление грыж межпозвонкового диска на уровне L4–L5, L5–S1. На уровне L3–L4 по данным магнитно-резонансной томографии (МРТ) выявлена грыжа диска, по данным функциональной рентгенографии – признаки нестабильности. Для предотвращения дальнейшего прогрессирования дегенеративного каскада пациентке проведено оперативное вмешательство – динамическая транспедикулярная фиксация с применением стержней из нитинола – металла с эффектом памяти формы. До и после операции проводили клиническую оценку (по шкале боли ВАШ, анкете оценки качества жизни Освестри, модифицированной шкале Macnub), МРТ поясничного отдела позвоночника (анализ степени дегенерации фиксированного и смежных сегментов по классификации Пфирмана), функциональную рентгенографию (оценка объема движений в фиксированном и смежном сегментах).</p> <p><bold>Результаты. </bold>Наблюдали значительное снижение боли в спине, полный регресс боли в ноге и улучшение качества жизни по сравнению с дооперационным уровнем.
