First group included 94 patients with pregnancies; the second group consisted of 110 patients, who failed to conceive within a stated period of time; in the third group there were 50 healthy women with normal pregnancies. Probability of developing pregnancy complications, time and method of delivery, anaesthetic procedures, weight, and height of newborns were assessed, while in the groups of patients with MS the risk of exacerbations and severity of the complications were evaluated.</p> <p><bold>Results and discussion.</bold> In the first group, there were more frequent threats of miscarriage and preterm birth, which might have taken place due to a complex of factors (cancellation of DMTs, use of hormonal therapy for exacerbations during pregnancy). The high frequency of caesarean section in the first group was associated with the unreasonable alertness of obstetrician-gynaecologists and the fear of the patients that the course of MS would worsen. Frequent exacerbations during pregnancy were associated with the abolition of DMTs before pregnancy and the failure of physiological immunosuppression during pregnancy.</p> <p><bold>Conclusion</bold>. In Russia, there is no single protocol for managing patients with MS during the period of family planning, pregnancy, and the postpartum period, which causes certain difficulties. There is emerging evidence that certain DMTs can be prescribed during pregnancy and lactation, which will help minimize the risks of exacerbations and disability increase.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение. </bold>Планирование и течение беременности у пациенток с рассеянным склерозом (РС) влечёт множество вопросов и требует комплексного подхода неврологов и акушеров-гинекологов.</p> <p><bold>Целью</bold> исследования было изучение особенностей течения беременности и родов у пациенток с РС.</p> <p><bold>Материалы и методы. </bold>В исследование включены 204 пациентки с ремиттирующим РС умеренной активности, планирующие беременность и принимающие препараты, изменяющие течение РС (ПИТРС), 1-й линии до наступления беременности. У 94 пациенток (1-я группа) за период наблюдения планируемая беременность наступила, а у 110 пациенток (2-я группа) — не наступила. Контрольную группу составили 50 здоровых женщин (3-я группа) с физиологически протекающей беременностью. Оценивали вероятность развития осложнений беременности, срок и метод родоразрешения, анестезиологическое пособие, массо-ростовые показатели новорожденных, вероятность развития обострений и их тяжесть у пациенток с РС.</p> <p><bold>Результаты и обсуждение.</bold> В 1-й группе чаще отмечались угрозы выкидыша и преждевременных родов, что, вероятно, связано с комплексом факторов (отмена ПИТРС, применение гормональной терапии обострений в период беременности). Высокая частота кесарева сечения в 1-й группе связана с необоснованной настороженностью акушеров-гинекологов и боязнью пациенток ухудшения течения РС. Частые обострения в период беременности связаны с отменой ПИТРС до беременности и несостоятельностью физиологической иммуносупрессии в период беременности.</p> <p><bold>Заключение.</bold> В России нет единого протокола ведения пациенток с РС в период планирования, во время беременности и в послеродовом периоде, что вызывает определенные трудности.
