Intrathecal synthesis indicators were measured in all patients in paired samples of cerebrospinal fluid and serum.</p> <p><bold>Study results. </bold>In the study group, the MRZ reaction was a specific test for the diagnosis of MS and was not found in neuromyelitis optica or acute disseminated encephalomyelitis. An increase in the IgG synthesis index was observed in only 90.5% of patients with MS, and oligoclonal IgG was detected in only 71.4% of cases. Elevated albumin quotient values are not typical for MS, which characterizes the degree of permeability of the blood-brain barrier.</p> <p><bold>Conclusion.</bold> The MRZ reaction is a highly specific laboratory marker for verifying MS. It also enables differential diagnosis of MS and other demyelinating diseases.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение.</bold> В основе аутоиммунного воспаления при рассеянном склерозе (РС) лежит поликлональная активация В-лимфоцитов в центральной нервной системе с последующей интратекальной продукцией иммуноглобулинов, которая обусловлена синтезом антител к вирусам кори, краснухи и ветряной оспы (MRZ-реакция).</p> <p><bold>Цель</bold> исследования — оценить возможность использования индекса IgG, олигоклонального синтеза иммуноглобулинов, коэффициента проницаемости гематоэнцефалического барьера, специфических индексов антител против вирусов кори, краснухи и ветряной оспы с расчётом MRZ-реакции в диагностике РС, оптиконейромиелита и острого диссеминированного энцефаломиелита.</p> <p><bold>Материалы и методы.</bold> В исследование включены 40 пациентов с диагнозами: РС (<italic>n</italic> = 19), острый диссеминированный энцефаломиелит (<italic>n</italic> = 11), оптиконейромиелит (<italic>n</italic> = 10). Показатели итратекального синтеза определяли у всех пациентов в парных образцах цереброспинальной жидкости и сыворотки крови.</p> <p><bold>Результаты исследования. </bold>В обследованной группе MRZ-реакция является высокоспецифичным тестом для диагностики РС и не встречается при оптиконейромиелите и остром диссеминированном энцефаломиелите, в то время как повышение индекса синтеза IgG отмечается только у 90,5% пациентов с РС, а олигоклональный IgG был обнаружен лишь в 71,4% случаев.
