The dissection presented as ischemic stroke in 175 patients and with local symptoms in the others. Out of 173 patients, 156 were managed conservatively. Efficacy of the conservative treatment in first 3 months was assessed in 143 patients who received anticoagulants (AC; <italic>n</italic> = 38), antiplatelets (AP; <italic>n</italic> = 42), both (<italic>n</italic> = 45) or no antithrombotic therapy (<italic>n</italic> = 18).</p> <p><bold>Results. </bold>Good functional recovery (0–2 points on the modified Rankin Scale) after 3 months was noted in 70% of patients, with no statistically significant differences between the different treatment types. There were 3 (2%) deaths in patients with severe stroke receiving AC. Recurrent ischemic stroke occurred in 10% of patients most often (86%) occurring in the first 3 weeks and triggered by head movement, straining, or fluctuating blood pressure. No statistically significant differences were found between different conservative treatments. However, there was a tendency towards increased frequency of recurrent stroke in patients without antithrombotic therapy (22%), as compared with patients receiving AC (4%) or AP (12%). There were no strokes in 110 patients with local symptoms due to ICA/VA dissection during the observation period.</p> <p><bold>Conclusion.</bold> There were no statistically significant differences in AC and AP efficacy in patients with ICA or VA dissection. A neck brace to prevent head movement and control of blood pressure are advisable alongside pharmacological therapy.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Цель</bold> работы — анализ результатов лечения больных с диссекцией внутренней сонной (ВСА) и позвоночной артерий (ПА).</p> <p><bold>Материалы и методы. </bold>Обследованы 285 больных (122 мужчины и 163 женщины, средний возраст 37,7 ± 8,1 года) с диссекцией ВСА (<italic>n</italic> = 147) и ПА (<italic>n</italic> = 122) или их сочетанием (<italic>n</italic> = 16). У 175 больных диссекция проявилась ишемическим инсультом, у остальных — локальными симптомами. Консервативно лечились 156 из 175 больных, из них эффективность консервативного лечения в первые 3 мес была оценена у 143 больных, получавших антикоагулянты (АК; <italic>n</italic> = 38), антиагреганты (АА; <italic>n</italic> = 42), их сочетание (<italic>n</italic> = 45) и не получавших антитромботическое лечение (<italic>n</italic> = 18).</p> <p><bold>Результаты. </bold>Хорошее функциональное восстановление (0–2 балла по модифицированной шкале Рэнкина) через 3 мес отмечено у 70% больных, статистически значимых отличий при разных видах лечения не выявлено. Летальный исход наступил у 3 (2%) больных с тяжелым инсультом, получавших АК. Рецидивы ишемического инсульта наблюдались у 10% больных, большинство из них (86%) происходили в первые 3 нед и провоцировались движениями головы, натуживанием, колебанием артериального давления. Статистически значимых различий при разном консервативном лечении не отмечено, хотя имелась тенденция к большей частоте рецидивов у больных без антитромботического лечения (22%), чем при приеме АК (4%) или АА (12%). У 110 больных с диссекцией ВСА/ПА, проявившейся локальными синдромами, инсультов за время наблюдения не было.</p> <p><bold>Заключение.</bold> Статистически значимые различия в эффективности применения АК и АА при диссекции ВСА и ПА отсутствуют.
