The ischaemic stroke subtype, stroke severity, level of functional dependence, and level of daily and motor activity in patients were established.</p> <p><bold>Results.</bold> In our group, women were older than men by 4 years on average (<italic>p</italic> = 0.018). The mean female age was 67 years (58–71 years), while the mean male age was 63 years (58–66 years). Women were more often diagnosed with cardioembolic stroke, while men were more often diagnosed with an atherothrombotic stroke. Ipsilateral internal carotid artery stenosis &gt;50% and left ventricular hypertrophy were more common in men than women, while left atrial dilation was more common in women. No significant differences were found between the two genders in the clinical presentation, acute stroke severity, and functional brain recovery.</p> <p><bold>Conclusion.</bold> In middle and older age patients, gender does not affect ischaemic stroke morbidity and rate of functional brain recovery by the end of the acute stroke period.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение.</bold> Современные исследования направлены на поиск возрастных и гендерных особенностей заболеваемости и смертности, факторов риска, причин развития, тяжести течения, характера восстановления утраченных функций мозга при ишемическом инсульте (ИИ).</p> <p><bold>Цель </bold>исследования — изучить влияние пола на основные клинические характеристики ИИ в бассейне артерий каротидной системы у пациентов в возрасте 45–74 лет, которым не проводились тромболизис и механическая тромбэктомия.</p> <p><bold>Материалы и методы.</bold> Обследованы 124 пациента: 77 (62,1%) мужчин и 47 (37,9%) женщин среднего и пожилого возраста с ИИ в бассейне артерий каротидной системы, поступившие в ФГБНУ НЦН в течение первых 48 ч с момента развития неврологической симптоматики. Устанавливали подтип и тяжесть ИИ, степень функциональной зависимости пациентов, уровень повседневной и двигательной активности.</p> <p><bold>Результаты.</bold> Женщины были старше мужчин в среднем на 4 года (<italic>p</italic> = 0,018). Медиана возраста женщин составила 67 лет (58–71 год), медиана возраста мужчин — 63 года (58–66 лет). У женщин чаще диагностировали кардиогенный эмболический ИИ, у мужчин — атеротромботический. У мужчин чаще по сравнению с женщинами выявлялся стеноз внутренней сонной артерии более 50% на стороне инфарктов мозга и гипертрофия левого желудочка, среди женщин — дилатация левого предсердия.
